目前从外科本身的角度看， 仍然有许多需待改进的问题，术中如何处理肺引流区域淋巴结就是其中之一。因为纵隔淋巴结转移率在NSCLC的患者中约占20%-40%，是预测患者预后的重要因素之一，那么在切除肺的原发肿瘤的同时行系统性肺门纵隔淋巴结清扫术是否可取得更好的生存率？这一问题一直是肿瘤外科界争论的焦点，有些学者认为在所有肺癌患者的外科手术中应行系统性肺门纵隔淋巴结清扫术[1]，标准的外科治疗实体肿瘤应该行区域淋巴结清扫，这种理念认为原发肿瘤增长和播散是通过区域淋巴结最终到重要器官[2,3]；亦有学者认为为减少围手术期的合并症和死亡率应选择纵隔淋巴结摘除术。有研究[4]回顾性分析认为淋巴结清扫不能够提高患者的总体生存率，淋巴结的切除应涉及分期、预后及区域控制的因素。产生这些不统一的主要原因在于所获...
