The definitive diagnosis of cocaine induced pulmonary emphysema was established after the concentration of [alpha]1-antitrypsin was found to be within the normal range. Besides congenital [alpha]1-antitrypsin deficiency, cocaine smoking should be considered as a possible cause of severe premature bullous emphysema.
